Graduation Information Request
ATTENTION: Class of 2017 Graduates
We need information from our graduating High School Seniors for Graduation Recognition Sunday on May 21st at 10:30 am!
Please fill out the form below to be a part of this special day in our church! We will also need for you to submit your picture (head shot works best- formal/informal senior photo) to Theresa Parker by emailing it to email@example.com. If you do not have an electronic copy, please bring the original photo to the church office and we can scan it for you.
All information and Senior Photo must be submitted by May 7, 2017.
Graduate Luncheon- May 21st
We will have a luncheon in the Student Center immediately following the Graduate Recognition in the 10:30 service.
There will be a special photo presentation, so please indicate if you would like to provide additional photos (limit of 5, submit by email to firstname.lastname@example.org by 5/14. These can be baby pictures, sports photos over the years, family photos, school pictures, prom, church camp photos, etc.
This celebration is open to graduates and their immediate family. If you will be joining us, please complete the information below by MAY 7th so that we have good numbers for planning, set-up, food, etc. Thank you!

Summer Camp (SWO) dates for students in grades 7-12 are July 24-29.
"Early Bird"- Total Cost: $335 each, if paid in full by March 1st.
Total Cost After 3/1: $385 each, must be paid in full by June 4th

Be sure to select the fund "CAMP:SWO 2017" so that your event payment will be applied correctly!
PLEASE NOTE: AFTER MAKING YOUR PAYMENT, PLEASE FORWARD THE CONFIRMATION RECEIPT (THAT YOU WILL RECEIVE BY EMAIL) TO firstname.lastname@example.org.
PAYMENTS BY CASH / CHECK
If you wish to pay by cash or check, payments may be given to Mrs. Theresa on Wednesdays, Sundays OR bring by the church office during regular business hours. Please be sure that cash payments are in an envelope with the student's name and event clearly marked. Checks can be made out to FBC Lexington.
